The Semi Annual Certification  form is the Activity Report for Employees Working on a Single Cost Objective. This form is required to report to the granting agency. that the percentage of time personnel works on the grant objectives.

The Time and Effort form is important in keeping track of the time grant personnel spend on performing the duties of the grant. It is important that this form is filled out on a daily or weekly basis because it is auditable by the agency awarding the grant.
