Students who are enrolling for Early Childhood Education (PK or Head Start) must have an SRQ completed per child.  If question 1 ( Do you rent, lease of own a house or apartment) is answered as "NO", the SRQ must be faxed or emailed to the Transitions Program.  Fax number is on the bottom of the form.
