Registration info for 2012-2013

For Returning Students please bring the following items:
1.  proof of residency (SAWS bill, CPS bill, home phone bill or lease with parent's name and address will be accepted)  
2.  proper identification of legal parent/guardian.

For New Students please bring the following items:
1. previous school's report card
2. social security card
3. birth certificate
4. immunization record
5. proof of residency (SAWS bill, CPS bill, home phone bill or lease with parent's name and address will be accepted)
6. proper identification of legal parent/guardian
