The SAISD Early Childhood Department will continue taking applications for its Head Start
program at any Head Start site. Just ask for the Family Support Worker in the Main Office.
Please be prepared to provide the following documentation when you come in to
apply:
- Proof of Age for child
- Social Security Card
- Immunizations
- Previous Year's Tax Return or Proof of income for previous 12 months
- Current Proof of Residence
- Photo ID for Parent
- Current Physical for Child
- Insurance Card

- Disability Documentation
- CPS Documentation
