An Unofficial Service Record is a snapshot of the employee's service record; it does not include the full-service record. Unofficial Service Record is used for education/certification purposes only.
Unofficial Service Records:
Submit your request via email to Service Record Request
In the body of your emailed request, include the following information:
- First and Last Name:
- Employee ID:
- Last 4 of SS:
- Purpose for the request:
Processing Time: 7-15 Business Days
All request are processed in the order received. Notification of completion will be sent via email.
